Semaglutide is in a class of drugs that have been used for over a decade in the treatment of type 2 diabetes, these are called GLP1-RAs (glucagon-like peptide 1 receptor agonists). Healthcare has noticed the weight loss effects of these medications for quite some time however recently this type of medication was tested on individuals who were not diabetics, and the results were so astonishing the FDA has approved Semaglutide for weight loss under the brand name Wegovy®.
How well does Semaglutide work?Low dose Semaglutide (1mg) once weekly resulted in an average weight loss of 11 pounds in 12 weeks. One 20-week, phase II trial looked at the effects of Semaglutide 2.4mg weekly on gastric emptying, appetite and energy intake. Patients receiving this dose resulted in loss of 9.9% of their bodyweight compared with 0.4% in those receiving a placebo.
High dose (2.4mg) Semaglutide studied in non-diabetic, obese patients resulted in more than 50% of the participants in the STEP trial loosing 15% of their body weight and between 33-40% of participants lost 20% of their body weight. Why is this so impressive? This number is higher than any results with the current best weight loss agent (phentermine with topiramate which resulted in approximately 10% weight loss in one year).
How does Semaglutide work?Reducing secretion of glucagon Semaglutide causes gastric emptying to be delayed which mimics “feeling full” leading to reduced calorie intake. This delay of gastric emptying also reduces appetite and food cravings.
How is Semaglutide administered?Once weekly injections into the subcutaneous tissue “fatty tissue” located on the abdomen, back of arms or upper thighs. The needle used is very small and most people do not feel the injection. There are advantages to once weekly injections, you do not have to remember daily medications and the Semaglutide is working 24/7 suppressing your appetite without the stimulant effects of other weight loss medications.
Are there side effects?The most common side effects are GI (stomach/intestines) related and include nausea, vomiting, constipation, stomach pain, headache, tiredness (fatigue), diarrhea, feeling bloated, belching, gas, dyspepsia (heartburn). It should be noted that most of these are manageable and many overlap with the intended effect of “feeling full”.
Am I a candidate for Semaglutide?There are only two qualifications for Semaglutide:
A BMI over 30 and no weight related conditions
A BMI over 27 with weight related complications
You can check your BMI online but a BMI of 27 roughly corresponds to a 180 pound 5’8” person and a BMI of 30 is around 200 pounds in the same person. If your BMI does not meet, you can still qualify if your body composition reveals excess body fat.
WHO CANNOT TAKE SEMAGLUTIDE?There are a few, rare situations where Semaglutide is absolutely contraindicated. If you or anyone in your immediate family has had medullary thyroid carcinoma or if you have Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).
Does insurance cover Semaglutide?Most insurance or Medicare plans DO NOT cover this medication, but in some cases manufacturer and pharmacy coupons can help offset the cost, currently it is very expensive (~$1300 a month).
